SIJ Special Test Cluster

Examination of the SIJ is difficult. There are a multitude of clinical test that show limited reliability and validity on their own. A cluster of SIJ provocation tests can be used as a more reliable system to identify the SIJ as a driver for symptoms.
The cluster includes SIJ compression, distraction, thigh thrust, sacral thrust and Gaenslen's tests. When 3 of 5 tests are positive it suggests there is a high probability of SIJ pain present. This cluster has been agreed as an acceptable clinical tool to make reliable predictions of sacroiliac joint pain.
SIJ Assessment
Gaenslen's Test
Positive when patients SIJ pain is reproduced
Compression Test
Positive when patients SIJ pain is reproduced
Distraction Test
Positive when patients SIJ pain is reproduced
Thigh Thrust Test
Positive when patients SIJ pain is reproduced
Sacral Thrust Test
Positive when patients SIJ pain is reproduced
When 3 or more tests are positive then there is a high probability of sacroiliac joint pain present
When less then 3 tests are positive there is less probability sacroiliac joint pain is the driver
